JVS provides the coordination of healthcare services to a vulnerable population of adults under age 55 residing throughout Northern New Jersey, who have severe disabilities and numerous physical, developmental, or psychological health issues. A Healthcare Case Manager serves as a liaison between these individuals working at the JVS Joseph L. Weinberg Rehabilitation Center (JLWRC) and the healthcare system. This program, funded by a grant from the Healthcare Foundation of New Jersey, also offers healthcare services, basic healthcare screenings, and on-site educational trainings for JVS clients. JVS also provides an annual Health and Wellness Fair for all its clients.
